The management of cleft lip and palate

Great Ormond Street Hospital for Children, Great Ormond Street, London, WC1N 3JH, UK, St Andrew's Centre for Plastic Surgery, Broomfield Hospital, Chelmsford, Essex, CM1 7ET, UK

Abstract

Clefts of the lip and palate are best managed by teams of specialists including surgeons to undertake primary and secondary surgery, orthodontists, speech and language therapists, ENT surgeons, audiologists, clinical nurse specialists, paediatricians, psychologists and paediatric and restorative dentists. The paediatrician has a vital role in diagnosis and in management of the neonatal period, taking an overall view of the patient's and family's needs, advising regarding associated conditions and monitoring general development.

The quality of outcome of treatment varies greatly between centres and appears to depend very much on the expertise of these team members, and especially on the quality of primary surgery. There are therefore good reasons for centralizing care so that all team members can develop appropriate experience.
