The management of persistent pulmonary hypertension of the newborn

Neonatal Intensive Care Unit, John Radcliffe Hospital, Headington, Oxford OX3 9DU, UK

Abstract

Persistent pulmonary hypertension of the newborn (PPHN) is a potentially devastating condition affecting infants of all gestational ages. There is no single effective therapy and there is a significant associated mortality. This review outlines the underlying pathophysiology and essentials of diagnosis and explains current therapies. Management is dependant on attention to detail with regard to general principles in combination with specific therapy. Specific treatment with vasodilators, nitric oxide and extracorporeal membrane oxygenation (ECMO) are elucidated and novel therapies touched on.
